At present there is no reliable method to determine which patient has constitutional delay in growth and which has total and complete gonadotrophin deficiency until several years of observation have passed. In general, if puberty has not started spontaneously by the age of eighteen years, the patient probably has GnRH deficiency, although there is at least one published case of a patient who spontaneously entered puberty at twenty-five years of age.
